It does seem like his nipple has been removed. I saw a picture last week with a guy who had his nipples removed but pierced them after they were gone. And I think that they did put red into that koi tattoo.

the bruise is from a dermal punch done on the inside of my arm for a biopsi as i had some skin irritation – long before ink.. it now seems like a reaction to some insect byte….
